If you lost your paycheck, you need to fill out a Stop Payment Request Form. There is a charge of *$30.00* for lost check. Once we receive confirmation from ADP of stopping your check (3 days the most), we will let you know when the replacement check is available.
To change your address you must complete an Staff Status Form. Check the Change of Address box and Complete with your updated information. The request will be effective approximately 2 weeks after the request is turned into the Payroll department.
The Foundation now offers Direct Deposit to all employees. To have your check directly deposited on payday to your bank account complete the Direct Deposit Form, attach a voided check, and submit to the Payroll office. On average it will take 2 pay cycles for your direct deposit to take effect.
