New Access

Electronic System Access Request Form

"Request for access to a CSUEB application, system or shared network drive. You can also request connections to a CSUEB computer from on or off campus (VPN).

For access to systems or applications that require formal approvals, a request must be submitted from the Electronic System Access Request Form (eSARF) application and required information security training must be completed.

Required information security training includes the CSU's Security Awareness Training and Family Education Rights and Privacy Act (FERPA) training ( In addition, access to systems that contain medical data requires completion of Health Insurance Portability and Accountability Act (HIPAA) training."

Account and Access Removal

Terminate Access to All or Specific Accounts

"Termination of access to a particular system (please specify) or the disabling of access to all systems. Requests for normal termination of access are handled in a routine manner. In general, there is a waiting period of 15 days before any action is taken. After that period, access is removed or is set to a self-service level, or accounts are disabled. For students, self-service access is continued for two quarters, while they are eligible to register for classes. For Emeriti faculty members, e-mail access is maintained indefinitely.
