Application Procedures for ALP Programs
Use the checklists below to determine which documents you need to submit as part of your application. Download the required documents for your program, complete them and submit by mail or fax. Your application cannot be processed until all required documents are received.
For Intensive English and Visiting Student Programs
- Intensive English/Visiting Student Application Form (PDF, 217k)
- Nonrefundable $100 application fee
Submit a check, money order, or credit card payment of $100 US dollars payable to "CSUEB". ALP accepts Visa, Mastercard, and American Express credit cards.
- Declaration of Finance for F-1 Students (PDF, 109k)
- Verification of Financial Declaration (bank statements, letter, or seal)
- Signed Health Insurance Compliance form (PDF, 24k)
- Visa Information Form (PDF, 27k)
- Permanent Address Form (PDF, 101k)
- A Legible Copy of your Passport (name page and photo)
- Transfer Form, if transferring from another US university or school (JPG, 500k)
For Pre-MBA, Diploma, and Certificate Programs
- Pre-MBA, Diploma, and Certificate Application Form (PDF, 90k)
- Nonrefundable $100 application fee ($50 for Hospitality Career Certificate)
Submit a check, money order, or credit card payment of $100 US dollars payable to "CSUEB". ALP accepts Visa, Mastercard, and American Express credit cards.

- Verification of Financial Declaration (bank statements, letter, or seal)

- Official TOEFL Score
- Transfer Form, if transferring from another US university or school (JPG, 500k)
Additionally, all Diploma and Certificate programs except the Hospitality Career Certificate require the following:
- Official transcript verifying completion of 4-year bachelor degree or equivalent
Visa Information
Students admitted to ALP Programs from abroad will receive the I-20 Form (Certificate of Eligibility) so that a student visa may be obtained at the nearest United States Consulate.
Students transferring from another institution in the United States will receive the I-20 Form which must be presented for special processing on the first day of the quarter at the Registration meeting.
Withdrawals and Refund Policy
- No refund of application fees.
- 100% refund of all tuition fees paid before the beginning of classes.
- 65% refund if application for refund is approved before the end of the first week of classes.
- No refund after the first week of classes.
