Students must upload their vaccination certification or request an exemption by Sept. 15. 

  1. Access MyCSUEB
  2. Click on the Classic View Icon.
  3. Under the “Student” tab, select COVID-10 Self Certification.
  4. Select the vaccination status and provide a digital copy of your vaccination record, as applicable.
