Youth Program Registration

Youth Program Directors must register their Youth Program annually by submitting the Youth Program Registration Form at least 45 calendar days prior to the start of any program activities involving minors.

Risk Management will review all submissions and may request additional information as part of the approval process. All programs must receive Risk Management approval before any activities involving minors may begin. 

Failure to complete registration and obtain approval in a timely manner may delay program start dates.

 

Before You Begin

Be prepared to provide:

  • Program name and description
  • Program type (CSUEB, Auxiliary, or External Organization)
  • Sponsoring department or auxiliary
  • Program location(s)
  • Program dates and times
  • Estimated number of participants by age group
  • Number of chaperones
  • Program Director contact information
  • Whether transportation will be provided
  • Whether overnight stays are planned
  • A list of program activities, including any free-time activities
  • Information about any co-sponsoring organizations

Participant Accident Insurance (PAI)

Depending on the number of participants and the types of activities involved, Participant Accident Insurance (PAI) may be required. Risk Management will review your program and notify you if PAI coverage must be purchased by the sponsoring department or organization.

 

How to Submit the Youth Program Registration Form

  1. Log in to Adobe Sign.
  2. From the Home page, select Start from Library.
  3. Select the Workflows tab.
  4. Search for "Youth Program Registration Form" and select Start.
  5. Enter the email addresses of the individuals the form should be routed to.
  6. Select Send Now.
  7. Complete and submit the form.
Contact Risk Management if there are significant changes to your approved program.
