Documentation
Our first priority is to meet with you and discuss your academic needs. You do not need formal documentation to schedule your initial Welcome Meeting. However, documents such as medical records, psychoeducational evaluations, or school records (such as an IEP or 504 Plan) can help guide the conversation and inform your accommodation needs. If you have these documents, please email them to as@csueastbay.edu when scheduling your meeting or provide them at the time of the Welcome Meeting.
Your AS Counselor may request new or updated documentation after your Welcome Meeting if a student's disability related barriers are not readily apparent.
We also have a Disability Information Form that can be completed by your medical or mental health provider.
