Consultation Request Form	For use by interested lineal descendants and tribes to arrange a consultation of a NAGPRA collection(s) held at Cal State East Bay.
Anonymous Reporting of Potential NAGPRA Finds at Cal State East Bay	This form is provided to allow anyone to anonymously report potential Native American Human Remains or Cultural Items that they may have encountered at CSU, East Bay.
Appeal a Cal State East Bay Determination	Please use this form to confidentially initiate an appeal regarding a campus determination regarding identification, repatriation or transfer of Human Remains or Cultural Items.
Report Repatriation Issues or Concerns at Cal State East Bay	Please use this form to confidentially report concerns regarding alleged violations of repatriation laws, alleged violations of the CSU systemwide NAGPRA policy, CSU East Bay personnel, or other NAGPRA related campus concerns.
